Number of Chaperones:  4 staff members (No parents)

Lunch:  We will have lunch at Pigeon Forge Snow Tubing. Your child can bring a bag lunch or request one form the cafeteria. Please note this on the form below.

Purpose of this trip:  5th Grade Students that have earned to go on a Reward Trip and celebrate the end of the 2nd semester.

VERY IMPORTANT INFORMATION -

Students will NOT be allowed to participate without having the digital waiver and this form completed.

Participants are REQUIRED to have a digitally signed waiver form. Must be 18 or older to sign the waiver. Anyone under 18 must be signed by parent.

• If you are unable to print out the confirmation email after you complete the wavier.
